The Community Health Center of the New River Valley has been recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). The NCQA works to improve health care through a process that uses measurement, transparency, and accountability to highlight top performers and drive improvement.

Among other requirements as an NCQA recognized program, we are committed to:

• Function in a manner consistent with the Joint Principles for Patient Centered Medical Homes focusing on team-based care led by an eligible clinician. 

• Provide a consistent level of excellence and commit to continuous monitoring. 

• Immediately communicate any material changes in the structure or operation of the practice or merger, acquisition, or consolidation.

The NCQA recognition process is challenging, and this accomplishment recognizes the exceptional work of our staff and the vision of our board and management to ensure quality health for our patients.

NCQA PCMH Recognition for Distinction in Behavioral Health Integration

Often, behavioral health conditions are identified by a primary care provider. Integrating behavioral health care services in the primary care setting is an opportunity for patients. It breaks down barriers to behavioral care and improves overall health. The National Committee for Quality Assurance’s (NCQA) Distinction in Behavioral Health is the ‘gold star’ for practices that have successfully integrated behavioral health services. 

According to the NCQA, the Distinction in Behavioral Health Integration “helps practices provide comprehensive whole person care that acknowledges the behavioral health needs of the individual beyond the core requirements of NCQA PCMH Recognition. The Distinction in Behavioral Health Integration calls for a primary care setting to have a care team in place to manage the broad needs of patients with conditions related to behavioral health, focusing on the use of evidence-based protocols and ongoing quality measurement and improvement. Integrating behavioral health into primary care can improve access, clinical outcomes and patient satisfaction.” 

The Community Health Center of the New River Valley integrates several healthcare services including behavioral health, addiction/recovery, oral health, nutrition counseling and more into primary care for a whole-patient approach to health and wellness. CHCNRV is one on only five health centers (out of 61) in Virginia to receive this distinction.
